Scrotal swelling can have many causes. Most are not life-threatening, and many are easily treatable. This post is meant to give you clarity and confidence — not fear — and encourage you to seek medical care that is respectful, private, and compassionate.

Hydrocele — A Common Cause of Scrotal Swelling

  •  Usually painless, but may feel heavy

  • Feels like a soft, water-filled balloon

  • Common in infants and older men

  •  May require a simple surgical procedure if large or uncomfortable

Hydroceles are benign and do not affect fertility — but like any swelling, they should be examined to rule out other causes.

Other Causes of Scrotal Swelling

Understanding the other possible causes can help you know when and why to seek care.

  1. Inguinal Hernia
  • Occurs when a part of the intestine pushes into the scrotum

  • The swelling often increases with standing or straining.

  • May cause a dragging sensation or discomfort.

  • Needs surgical correction — especially if painful or stuck (strangulated hernia).

  1. Epididymo-orchitis (Infection)
  • Infection of the testicle or the tube behind it (epididymis).

  • Presents with pain, swelling, fever, and redness.

  • May feel warm and tender to touch.

  • Treated with antibiotics — early treatment prevents complications.

3. Varicocele

  • Enlargement of veins in the scrotum (like varicose veins of the leg).
  • Feels like a bag of worms and may affect fertility.
  • Usually painless but may cause a dull ache or heaviness, especially by the end of the day.
  • Can be treated if causing symptoms or infertility.

4 .Testicular Torsion (Emergency)

  • This is when the testicle twists, cutting off its blood supply.

  • Causes sudden, severe pain and swelling — often in teenagers or young adults.

  • May be associated with nausea or vomiting.

  • Needs emergency surgery within 6 hours to save the testicle.

  • Don’t delay if pain starts suddenly — rush to a hospital or emergency clinic.

  1.   Testicular Trauma

  • Direct injury to the scrotum from sports, accidents, or falls.

  • Can cause bruising, swelling, bleeding inside the scrotum, or rarely, testicular rupture.

  • Some injuries are minor; others need urgent surgery.

  • If pain or swelling persists after an injury, it should be evaluated right away.

6.Testicular Tumors

  • May present as a firm, painless swelling in the testicle.

  • Often discovered accidentally by self-examination.

  • Early-stage testicular cancers are highly curable.

  • Regular testicular self-checks help in early detection.
